Official Name Change
Students who wish to change their legal name on their student records should use the following guidelines to request the change:
- Supporting Documentation is required for both changes to name and gender marker. You must be able to present current official government-issued documentation showing the correct spelling of your new name and/or gender marker. Accepted documentation can include a passport, driver’s license, or court order.
- You must complete the Change of Name Form. Completed forms can be sent via email to registrar@uchicago.edu. If you are mailing in your request, you may send it to:

- Graduating Students should submit any name change requests to the Office of the University Registrar at least 2 months before they are expected to graduate.
- Booth Students should refer to their Program Office for instructions on submitting a name change request.
- Alumni should also contact Alumni Relations to request a name change. Submitting a Change of Name Form to the Office of the University Registrar will not automatically update your alumni record.
- Student Employees: students employed by the University will also need to contact HR Services to update their employee record with their new name, as the information is stored on a separate system.

Legal | Name reported via admission application. Should match one’s legal name as it appears on state and federal documents. | Official and financial records: transcripts, financial aid, Bursar records (including tuition bills), diplomas, Workday/HR*. Legal names appear on Chicago Cards. The Office of International Affairs only uses legal names. The legal name is what appears in all systems by default, in the absence of a preferred name or legally changed name. |

Email display names and Student Wellness records are updated separately. Email aliases can be managed through MyAccount. Name and gender identity for Student Wellness can be updated through My.WellnessPortal. *Any students or alumni who work on campus and request a legal name change must also update their information in Workday. It is important to do this as soon as possible. If the information in Workday does not match our records, this may prevent the corrected name from displaying.
